Description
This report analyzes the coverage, benefits and expenditures and financing of the Peruvian Institute of Social Security (IPSS) medical care program against the background of Peru's continuing economic crisis that has substantially reduced the institute's revenue base. The report concludes by camparing Peru with other Latin-American countries that confront similar problems developing medical care under social secuirity programs and recommends ways to improve the program's current situation

    Pharmaceuticals in Peru

    Pharmaceuticals represent about 15-20 percent of total health sector expenditures in Peru. This proportion is lower than in most other countries, and the shortage of medicines and other pharmecutical supplies is a major constraint to the effective dilivery of services, especially through public health and social security programs. While about 35 percent and 12 percent, respectively, of the population have access to these two health care programs, only about 20 percent of all pharmeceuticals are distributed through public health and social security facilities. For the most part, Peruvians purchase their medications from private pharmacies. A long-standing attempt by the government in Peru to provide free or low-cost basic medicines to the population through public health and social security has been largely unseccessful. Private sector availability of pharmaceuticals, however, has also become increasingly difficult, due to the rising cost of imported materials and governmental price controls on products sold through private pharmacies. This technical report, prepared as part of the HSA-Peru, will review the evolution of low-cost, generic drug programs in relation to the importation and domestic production under price controls of pharmeceuticals in Peru

    Health accounts from past to present for a political arithmetic

    [ABSTRACT]. This report traces the progressive expansion of health accounts (HA) to measure national health expenditures, from the first attempts in 1926 by the American Medical Association to the present day. Milestones in the development of A System of Health Accounts (SHA) are covered, from the economic background to initial efforts by a few countries and organizations, to the need for a set of accounting standards for health care systems, and finally, to consolidation with SHA 2011. International organizations, such as the World Health Organization, Organization for Economic Cooperation and Development, Eurostat, the World Bank, and the United States Agency for International Development have been critical to expanding national HA exercises and ensuring that these are standardized, comparable, and become institutionalized. National efforts to track health expenditures have not only enriched collective results, but have become an important component of global leadership, informing policy the world over. More than 100 countries have created HA under the global standard and have gained a better understanding of health spending and financial flows. These results are key for monitoring progress toward national and global initiatives, such as the Sustainable Development Goals and Universal Health Coverage. Challenges remain to be tackled, among them institutionalization and quality of results. Social responsibility for improving data sources and expanding the generation and usability of health accounts are also needed.

    Cuentas de salud del pasado al presente para una aritmética política

    [RESUMEN]. Este informe describe el proceso de ampliación progresiva de las cuentas de salud para medir los gastos nacionales en salud, desde los primeros intentos de la Asociación Médica Estadounidense en 1926 hasta la actualidad. Se mencionan los hitos en la creación del Sistema de Cuentas de Salud , desde los antecedentes económicos y las acciones iniciales de unos cuantos países y organizaciones a la necesidad de un conjunto de normas de contabilidad para los sistemas de atención de salud y, por último, la consolidación con el Sistema de Cuentas de Salud del 2011. Varias organizaciones internacionales, como la Organización Mundial de la Salud, la Organización para la Cooperación y el Desarrollo Económicos, Eurostat, el Banco Mundial y la Agencia de los Estados Unidos para el Desarrollo Internacional, han sido fundamentales para ampliar los ejercicios nacionales de cuentas de salud y asegurar que estén normalizados, sean comparables y se institucionalicen. Las acciones nacionales para realizar un seguimiento de los gastos en salud no solo han enriquecido los resultados colectivos, sino que se han convertido en un componente importante del liderazgo mundial, al fundamentar las políticas en todo el mundo. Más de 100 países han creado cuentas de salud de conformidad con la norma mundial, y han logrado una mejor comprensión del gasto en salud y de los flujos financieros. Estos resultados son clave para vigilar los avances relativos a las iniciativas nacionales y mundiales, como los Objetivos de Desarrollo Sostenible y la cobertura universal de salud. Todavía quedan retos por delante, como la institucionalización y la calidad de los resultados. También se necesita responsabilidad social para mejorar las fuentes de datos, y aumentar la producción y eluso de las cuentas de salud.

    Financiamento das politicas de saude no Brasil

    The purpose of this article is to examine the structure of health financing and expenditure in Brazil, based on the different components and institutions that manage the sector. The main public sources of financial and tax resources for the sector are described and special reference is made to the national fiscal budget, the Social Security and Welfare Fund (FPAS), the Social Invesment Fund (FINSOCIAL), and other federal social funds and state and municipal budgets. Expenditure is analyzed both from the vantage point of the institutions involved in managing the resources- Ministry of Health, National Medical Care and Social Security Institute (INAMPS), state and municipal secretariats- and from the standpoint of existing health programs and subprograms (medical care, public health, etc). Public expenditure is also compared to private expenditure, in order to estimate total health disbursements in Brazil and their functional breakdown. The repercussions of the economic crisis of the eighties were responsible for significant cutbacks i resources. In this regard, the author analyzes the main adjustment strategies used, especially by the Ministry of Social Security, to deal with the reduction in available resources. Lastly, the article identifies the main problems of the present system of sectoral financing and discusses changes in the system which have been proposed by the VIII National Health

    Experiencia de un equipo médico de emergencia del IMSS durante el sismo en México en setiembre de 2017

    [RESUMEN]. El objetivo de este trabajo es describir las acciones desarrolladas por el equipo médico de emergencia (EMT por sus siglas en inglés) del Instituto Mexicano del Seguro Social (IMSS) durante la emergencia derivada del sismo del 7 de setiembre de 2017, el cual tuvo una magnitud de 8,2 grados con epicentro a 133 km al suroeste de Pijijiapan, Chiapas, México. Una vez realizada la evaluación inicial intersectorial de la emergencia, se activó el sector salud. Dentro de esta respuesta, y una vez determinado un número elevado de lesionados y un riesgo alto para la salud pública de la población afectada, el IMSS desplegó en la región un EMT capacitado y entrenado para apoyar en la respuesta de atención médica y quirúrgica de lesionados. Las acciones del EMT, conformado por especialistas en urgencias médico-quirúrgicas y desastres, incluyeron la atención médica de 252 pacientes, la gestión del traslado de 57 pacientes y la colaboración en la habilitación de un hospital provisorio. Para mejorar la respuesta en situaciones de desastres, se requiere seguir el proceso de integración, registro y capacitación de los EMT a nivel nacional e internacional.
