Description
Research is an essential component of public health emergency preparedness and response. Therefore, it is essential to be able to carry out research as soon as a health emergency begins and to ensure that it is carried out in an ethical manner. This Pan American Health Organization publication provides direction for strategies and guidelines on modifying ethical oversight processes in emergency situations, ethically monitoring ongoing research in the face of new evidence, and overcoming other ethical challenges that arise when doing research in public health emergencies. It also provides guidance so that the emergency use of unproven interventions outside of a research setting is carried out in an ethical manner. Finally, it includes the lessons learned in the Region of the Americas during the COVID-19 pandemic and recommendations resulting from a regional reflection exercise aimed at improving ethical preparation for conducting research during future health emergencies.

Ethics Guidance for the COVID-19 Pandemic, 25 June 2020(PAHO, 2020)The COVID-19 pandemic presents us with the moral urgency to conduct rigorous research as soon as possible to produce evidence about the safety and efficacy of interventions to treat or prevent COVID-19. However, as in previous emergencies, marked by high mortality and the absence of safe and efficacious treatments, interventions that have not been previously proven for COVID-19 are being offered outside of protocols of research with human subjects. Since the safety and efficacy of these interventions have not been proven yet, their risk-benefit profile is unknown. Yet in the exceptional circumstances of the pandemic, they are being offered as an attempt to advance access to interventions that may benefit patients. As recommended by the Pan American Health Organization and the World Health Organization, unproven interventions should be offered within research protocols, and specifically within randomized controlled trials capable of assessing safety and efficacy.Item Template and operational guidance for the ethics review and oversight of COVID-19-related research, 15 April 2020(PAHO, 2020)As part of the response to the COVID-19 pandemic, health research has proliferated. Clinical trials and other studies with human participants are being conducted to find efficacious treatments and vaccines for COVID-19. In this context, research ethics committees (RECs) must be prepared and organized to conduct a thorough ethics review of research, and have rapid and flexible procedures in place to do so efficiently, considering the tight timelines and needs posed by this health emergency. This document aims at providing guidance for RECs to develop standard operating procedures (SOPs) for the rapid review of human subjects research related to COVID-19. It supplements the prior Guidance and strategies to streamline the ethics review and oversight of COVID-19-related research.
