The author reviews the reforms in the Chilean public health system over the last 10 years. After a brief characterization of the system, some of the new policies and social and economic indicators are evaluated, and the role of the public and private sectors in guaranteeing medical services is described. Up until 1980 the Chilean public health system was highly centralized and consisted of two parts: the National Health System (SNS), which provided preventive care to the entire population and curative care for workers and the indigent, and the preferred supplier system (SPP), which provided curative care for salaried employees. With the creation in 1979 of the National Health Services (SNSS), the SNS was divided into 27 autonomous regional services capable of providing more specialized primary and tertiary care. The Ministry of Health was responsible for setting standards and health policies, and the National Health Fund (FONASA) was created to collect wage deductions for health, finance the respective regional services, and manage the SPP. In 1981 the SNSS was expanded no include the health insurance institutions (ISAPRES) which sold closed group plans, preferred supplier plans, and mixed plans. The aim of the new policies was to decentralize the provision of medical services, encourage collaboration of the private sector, and ensure free choice by the users: to intensify preventive measures

Item Los altos costos de la atención de salud en el Brasil(s.d.)An analysis is presented of the changes that have occurred in Brazilian health sector expenditures in recent years, and of the factors that have favored the excessive growth of curative care at the expense of health promotion- in particular, the balance between the public and the private sector, and the role the public financing, and foreign medical technology, among others. The analysis focuses on the medical-hospital system in Brazil, given its importance as an essential supplier of health services, and on the National Institute of Medical Care and Social Welfare (INAMPS), the agency that under writes the major portion of medical and hospital expenditures. The first section outlines the general features of the fee system that was adopted by the INAMPS for payments to health service suppliers, as well as the basic standards that govern the work of this agency in the context of the Brazilian health system. Notable in this regard are the policy changes adopted in the early 1980s with a view to reducing costs. The second section examines the high cost of Brazilian health care with a discussion of the following topics: the role of the physicians, the INAMPS fee schedule, high technology in medicine, behavior and health costs, choice of services, excessive rates of cesarean deliveries, and physicians and the choice of technology. Objetivo. El objetivo de este trabajo fue recabar y analizar la evolución de los egresos hospitalarios en Chile entre 1952 y el 2020, tanto a nivel nacional como en el subsistema público. Se identificaron patrones de cambio en relación con los hitos institucionales claves: la ley de medicina curativa en 1968, la creación del Sistema Nacional de Servicios de Salud en 1979 y la implementación de la Reforma para el Acceso Universal y Garantizado en el 2005. Método. Se construyó una serie histórica inédita a partir de fuentes oficiales del Ministerio de Salud y documentos institucionales. Se calcularon indicadores del número de egresos totales, tasas por mil habitantes y comparación con Estados Unidos. El análisis incluyó exploración gráfica y estimación de modelos de regresión segmentada. Resultados. Los egresos hospitalarios crecieron de manera sostenida, con dos fases de fuerte expansión del sector privado entre 1982 y1987, y entre el 2006 y el 2019. Por su parte, la provisión pública se ralentizó o disminuyó con la reforma de 1979 y con la reforma del 2005, aunque no con la 1968. Conclusión. El estudio aporta evidencia empírica sobre la trayectoria hospitalaria chilena en el largo plazo, y confirma la contraposición estructural entre los modelos de salud principales y las diferencias en sus reformas. The World Health Organization (WHO) recommends a companion of choice during labor and birth, to improve maternal and perinatal outcomes and women’s satisfaction with health services. To better understand the status of companion of choice in Latin America and the Caribbean (LAC), an online survey was conducted with members of a midwifery virtual community of practice and with key informants, aiming to identify: 1) existing regulatory instruments related to companion of choice in the countries where the members are practicing; and, 2) key characteristics of implementation of companion of choice, where regulation exists. Responses (n = 112) were received from representatives of 20 of the 43 countries of LAC. Respondents reported existence of a national policy or legislation in seven countries, ministerial norms or institutional protocols in five countries, and no existing policy/protocol in eight countries. Respondents from the same country often provided contradictory responses. Responses differed from information provided by ministries of health in a WHO-led global policy survey in 11 instances. These variations may reflect that midwives were not always aware of the national policy/guideline in their country. We propose that a more robust effort should be undertaken to understand the status of companion of choice for labor and birth in LAC countries, at national, regional, and local level, in public and private facilities. It is important to know if policies exist, at what level of the system, and if key stakeholders, maternity-care health providers, and women are aware of their existence. Efforts should also be made to understand barriers to implementing companion of choice.Item Controle do câncer do colo do útero na atenção primária à saúde em países sul-americanos: revisão sistemática(2022)[RESUMO]. Objetivo. Descrever as estratégias para prevenção e controle do câncer do colo do útero (CCU) na atenção primária à saúde (APS) na América do Sul. Métodos. Revisão de literatura em duas etapas: revisão documental em sites governamentais de Argentina, Bolívia, Brasil, Chile, Colômbia, Equador, Paraguai, Peru, Uruguai e Venezuela; e revisão sistemática da literatura nas bases LILACS, MEDLINE, Scopus, SciELO e Science Direct. Resultados. Foram incluídos 21 documentos institucionais (planos, guias de prática e diretrizes nacionais) e 25 artigos. Todos os países tinham taxas elevadas de morbimortalidade por CCU. Predominou o rastreamento oportunístico na APS, embora os documentos disponíveis sinalizassem intenções e estratégias para diagnóstico precoce e acompanhamento longitudinal dos casos suspeitos e confirmados, preferencialmente na rede pública. Todos os países adotavam uma concepção abrangente de APS, embora o processo de implementação estivesse em estágios heterogêneos e predominassem a focalização e a seletividade. Destaca- se pior acesso ao rastreamento para mulheres de regiões rurais ou remotas e para povos originários. A indisponibilidade de serviços de APS próximos às residências/comunidade foi uma importante barreira para o rastreamento do CCU. Conclusões. A fragmentação dos sistemas de saúde e a segmentação na oferta de serviços são obstáculos para a prevenção e o controle do CCU na América do Sul. São necessários programas organizados de rastreamento do CCU e a incorporação de busca ativa para realização do Papanicolaou via APS. A interculturalidade nas práticas e a formulação de políticas numa perspectiva interseccional são fundamentais para superar as iniquidades no controle do CCU nos países sul-americanos.
